  1. New Volunteer Application (1 sheet, double-sided)

          All volunteers must watch the Habitat Safety Video and read the Work Crew Safety Manual (PDF) (3 pages) before working on site. 

          You must acknowledge that you have done so by signing and returning the following Acknowledgement Form to the Construction Manager the first day you volunteer.

Please also read the following publications carefully.

    3.  Safety First (PDF) (1 page)

    4.  General Instructions for Volunteers (PDF) (1 page)

    5.  Adult Waiver of Liability Form (PDF) (3 pages) OR

         Under 18 Waiver of Liability Form (PDF) (4 pages, must be signed by the volunteer and a parent or guardian)
